Default Horror Story...

I've been dating a woman for a while now and we've been working our way up to having sex for the first time. It finally happened, but about 15 minutes in something horrible happened. I lost erection. This has happened to me twice in the past, but there was always a logical reason. My first time ever, nervousness I suppose... and the other was stress. It was the most degrading and horrible thing I could think of to happen because she had looked so forward to it, and the first time is something that cannot be taken back. Naturally she assumed it was her fault and I assured her that it had nothing to do with her, that something must have went wrong on my side. I was exhausted, I had only had about 3 hours of sleep the night before and this took place late into the night after. I had also worked that day a lot harder than usual. I'm hoping this might be the prime suspect. I was also slightly nervous because we built it up so much. My question is how can I prevent this? We plan on trying again and I want to go above and beyond to make up for such a rough start. I'm terrified it will happen again, and I know I shouldnt be because this will only elevate the problem I'm sure. I'm 24 years old, in fairly good health and shape, wide range in foods. Any suggestions would be great! Thanks.
